The Vital-Check form asks for your first name (optional), email address and answers about everyday routines, including food, sleep, movement, stress, daylight and supplements. The weight-goal question is optional. Please do not share diagnoses or medication details.

Form answers are processed for the Vital-Check you expressly requested. The detailed evaluation is sent by email after the required consent.

For the PDF guide, an email address is required and a first name is optional. The download appears after submission. We send any guide-related follow-up emails only with your separate, optional consent.

Email 2 is only a short, friendly check-in question, with no recruiting or partnership pitch.
